Case Management

Case Management

The main part of case management is for a case manager to come out to the senior’s home and assess what services they may be eligible for based on their situation (i.e. home delivered meals, chore services, in home care, transportation services etc.) Call 708-383-8060 if you need help determining what services you need.

Protective Services

Adult Protective Services is a small, but of course important, part of case management services. Adult Protective Services (APS) APS works to prevent abuse, neglect and financial exploitation of senior citizens and people with disabilities aged 18-59 living in our community. Money Management Services

For those older adults that need assistance paying bills, sorting mail, balancing their checkbook and more. Free for those that meet the income guidelines, there is also assistance for those who can pay privately. Find out more about Money Management here or call 708-383-8060.


Financial Assistance for Family Caregivers

Financial Assistance (or Gap Filling) is available for lower-income River Forest residents.

ADRD (Alzheimer’s Disease and Other Disorders) GAP Funds: The purpose of this funding is to meet the identified needs of the person with dementia and/or their primary caregiver to ensure their health, safety and welfare of the individual with dementia (Max amount per client is $1,500 per year)

CRC Gap Funds: The purpose of this funding is to provide services to assist caregivers with unmet needs to maintain a care recipient’s independence, safety, and well-being by providing limited financial assistance. (Max amount per client is $250 per year)

CRG Gap Funds: The purpose of these funds is to provide services to assist grandparents or other non-parent relatives of any age raising relative children (who are not more than 18 or who are 19-59 with a disability) (Max amount per client is $250 per year)
